Network Connection Request Form—Explanation of Terms

User Information

Name of the Primary User:
The Primary User field should be filled in with the name of the person who will be using the computer.
Department if Faculty/Staff or Resident Hall if Student:
This field should be filled in with the name of the UNL department that the Primary User works for if they are faculty or staff. If the Primary User is a student it should be filled in with the UNL residence hall they live in.
User's Telephone Number:
This field should be filled in with the office phone number of the Primary User if they are faculty or staff. If the Primary User is a student it should be filled in with their residence hall phone number.
User's E-mail Address:
This address field should be filled in with the electronic mail address of the Primary User. A copy of the request will be sent to this address if it is filled in.

Technical Contact Information

Contact's Name:
This field should be filled in with the name of the person who is the technical contact for the Primary User's department if they are faculty or staff.
Contact's Telephone Number:
This field should be filled in with the office phone number of the technical contact.
Contact's E-mail Address:
The E-mail address field should be filled in with the electronic mail address of the technical contact. A copy of the request will be sent to this address if it is filled in.

System Location

Building the System is Located In:
This Field should be filled in with the name of the university building that the computer is located in.
Room Number:
This field should be filled in with the room number where the computer is located.

System Information

Ethernet card#:
The Ethernet card# should be filled in with the 12-digit, hexadecimal ethernet address of the computer. A hexadecimal number consists of the only digits 1 through 9 and the lowercase letters a - f. An example number would be the following, 00409576d4ef. Sometimes you will find an ethernet card# of the form 4445xxxx0000. It is what is referred to as a loop-back number used for testing. It is commonly given when your computer is not connected to a live port or active network. If you are using winipcfg to obtain your network number make sure that the top pop box is set to Ethernet adapter and not ppp.
Is This A Replacement Card?
This should be answered yes if you are replacing a previously registered card.
If it is a Replacement please enter the old Card Number, IP Address, or registered Hostname:
If it is a replacement card you need to enter one of the following;
— The IP Address of the computer the card is going in. This number should look some thing like this, 129.93.xxx.xxx. The old ethernet card number.
—OR—
—The registered hostname which is the name IS has assigned to your computer. You should have received this information via email after your request was completed.
Operating System:
What operating system does your computer use? Macintosh, Windows (3.x, 95, 98, NT, 2000, or XP), OS/2, Unix, or Linux.
